NEW ADMISSIONS FOR 2010-2011

We want to welcome you, as parents/ guardians and students to Haili Christian School.  Since 1978 we have envisioned a school where boys and girls could attend and hear God’s Word and learn in the highest quality education environment possible.  Our prayer is that you will be a dedicated partner with us as we seek to serve God in this place.   (Administration, Faculty, and Support Staff)

ENROLLMENT FEES  (Non-refundable / Non-transferable)

     New Registration:     $165.00    (Fee must accompany application)          

     Re-Enrollment Fee:     $140.00    (Fee must accompany application)          

 Please fill out each form completely and include signatures of both parents or guardians unless sole custody is authoritative.  Attach payment form  (this reserves a space for your child)  

  SUBMIT THE FOLLOWING DOCUMENTS:

Failure to submit the above information or non-compliance of any state health requirements will forfeit admission to Haili Christian School until requirements have been met.  Please notify the school’s Admissions Secretary immediately of any complications or delays in attaining the necessary documents.

HAWAII STATE HEALTH REQUIREMENTS FOR ALL PUBLIC AND PRIVATE SCHOOLS:  

IMMUNIZATION RECORD AND PHYSICAL

A physical examination is required within 12 months before FIRST ENTRY into the Hawaii school system (private or public)  

Physical examination must be signed by a Provider:  Medical Doctor (MD), Doctor of Osteopathy (DO), Advanced Practice Registered Nurse (APRN), or Physician’s Assistant (PA) 

 Physical examinations must be performed within the United States  

Immunization record must be reviewed and signed or stamped by Provider (please have record updated before entry into school)

VARICELLA VACCINE (“CHICKEN POX” VACCINE)

 The “chicken pox” vaccine will be required for entry into preschool, kindergarten, and 7th grade  AND  for all students entering the Hawaii school system for the first time, regardless of age

 A documented history of “chicken pox”, signed by the Provider, may be substituted for the “chicken pox” vaccine requirement

  7th GRADE ATTENDANCE

    Documentation of receipt of the following vaccines will be required for 7th grade attendance:

    2 MMR vaccines

    3 hepatitis B vaccines

    1 or 2 chicken pox vaccine(s)  

TUBERCULOSIS (TB) EXAMINATION

Tuberculosis (TB) Examination is required within 12 months before FIRST ENTRY into the Hawaii school system (a student who has a valid TB examination for preschool entry does not need another TB examination to enter kindergarten) 

TB test and / or chest x-ray must be performed within the United States

TB examination must have Provider’s signature or stamp

2009-2010 TUITION RATES

3 Pre. – 4 Year old Kindergarten            $ 4,050 per year

5 Year-old Kindergarten -6th Grade      $ 3,500 per year

7th – 8th Grade                                         $ 3,800 per year

Tuition discounts for families with more than (1) one child enrolled:

 EARLY STUDENT WITHDRAWAL

Withdrawal prior to the end of the school year has tremendous impact on the school's mission and our ability to have a debt-free ministry that brings honor to our Lord Jesus. Therefore, it is very important that all of our families understand the financial impact of early withdrawal and that everyone involved honors the following guideline:

All student withdrawal dates on or before August 30, 2010 will incur a $300 (per student) withdrawal fee deducted from prepaid tuition, regardless of written notice.

If a student must withdraw from school after August 30, 2010, parents should notify the school in writing at least thirty (30) days in advance of the date of withdrawal. Early withdrawal also prorates the payment plan from 10 to 9 installments and increases the amount due per installment. If account is in arrears, you will be invoiced for the amount owing. The school may retain report cards until accounts are current.

RETURNED CHECKS

In the event your check is returned by the bank, you will be notified and a $20.00 return check service charge will be added to your payment due along with a 5% late fee. If the check (or replacement check) is returned once again, you will be expected to pay in cash or by cashier's check.

After School Care Program  (For Preschool – 6th Grade Only)

Regular  (Full month fee)                               $  120.00 per month

Occasional  (payable on day of service)        $  15.00 per day

 3 year-old preschool – 8th grade:      8:00 am  school begins (M – F)

                                                             2:15 pm dismissal  (M,T,TH,F)

                                                                         1:15 pm dismissal  (Wed)  

   UNIFORMS

Uniforms are mandatory at HCS and order forms are provided throughout the year. Uniforms consist of a the school's Polo or T-shirts     and pants in khaki or navy blue. The HCS dress code is to promote neatness and modesty.

TAX DEDUCTIBLE GIFTS

Tuition and fees cover basic educational costs, such as salaries, administrative costs, building and equipment maintenance utilities, classroom expenses and transportation.  

We encourage giving toward:  the general fund, scholarship fund, building fund, athletic fund, or classroom funds.  Contributions to HCS are tax-deductible and will be receipted.  

Our commitment continues to be the same:  providing the finest Christ-centered education possible which will encourage our children to renew their minds and be transformed.
