NEW ADMISSIONS FOR
2008-2009
We want to welcome you, as parents/ guardians and students to Haili Christian School. Since 1978 we have envisioned a school where boys and girls could attend and hear God’s Word and learn in the highest quality education environment possible. Our prayer is that you will be a dedicated partner with us as we seek to serve God in this place. (Administration, Faculty, and Support Staff)
ADMISSION FEES: NON-REFUNDABLE / NON-TRANSFERRABLE
Registration: Re-enrollment: $100.00 After March 30th: $150.00
New Student: $150.00
The
registration fee must accompany application in order to process enrollment.
Please fill
out each form completely and include signatures of both parents or guardians unless
sole custody is authoritative.
SUBMIT
THE FOLLOWING DOCUMENTS:
Failure to submit the above information or non-compliance of any state
health requirements will forfeit admission to Haili Christian School until
requirements have been met. Please
notify the school’s Admissions Secretary immediately of any complications or
delays in attaining the necessary documents.
HAWAII
STATE HEALTH REQUIREMENTS FOR ALL PUBLIC AND PRIVATE SCHOOLS:
IMMUNIZATION
RECORD AND PHYSICAL
A physical
examination is required within 12 months before FIRST ENTRY into the Hawaii
school system (private or public)
Physical
examination must be signed by a Provider: Medical
Doctor (MD), Doctor of Osteopathy (DO), Advanced Practice Registered Nurse (APRN),
or Physician’s Assistant (PA)
Physical
examinations must be performed within the United States
Immunization
record must be reviewed and signed or stamped by Provider (please have record
updated before entry into school)
The “chicken
pox” vaccine will be required for entry into preschool, kindergarten, and 7th
grade AND
for all students entering the Hawaii school system for the first time,
regardless of age
A documented
history of “chicken pox”, signed by the Provider, may be substituted for the
“chicken pox” vaccine requirement
7th GRADE ATTENDANCE
Documentation
of receipt of the
following vaccines will be required for 7th grade attendance:
2 MMR vaccines
3 hepatitis B
vaccines
1 or 2 chicken
pox vaccine(s)
TUBERCULOSIS
(TB) EXAMINATION
Tuberculosis
(TB) Examination is required within 12 months before FIRST ENTRY into
TB test and / or
chest x-ray must be performed within the United States

must have Provider’s signature or stamp

Payment is considered overdue if not received by the due date at which time a 5% late fee will be added to the total
Prices for student text books will be announced April 2008

UNIFORMS
Uniforms are mandatory at HCS and order forms are provided throughout the year. Uniforms consist of a the school's Polo or T-shirts and pants in khaki or navy blue. The HCS dress code is to promote neatness and modesty.
TAX
DEDUCTIBLE GIFTS
Tuition
and fees cover basic educational costs, such as salaries, administrative costs,
building and equipment maintenance utilities, classroom expenses and
transportation.
We
encourage giving toward: the
general fund, scholarship fund, building fund, athletic fund, or classroom
funds. Contributions to HCS are
tax-deductible and will be receipted.
